Referral & Intake System
At Cornerstone, our core work centers on providing individualized intervention services to those who need them most. To help us coordinate care thoughtfully and respond as quickly as possible, we ask that all referrals be submitted through our referral and intake system.
This system serves as the primary way for partners, staff, and individuals to submit referrals on behalf of a client or themselves. It allows our team to gather essential background information and informed consent upfront, so we can better understand each situation and respond with thoughtful, appropriate support.
